University of Dayton Information Report Form

This form is available for any UD and external community members to report incidents of bias or harassment or discrimination that you believe may be in violation of the University of Dayton's Nondiscrimination and Anti-Harassment Policy. The University takes reports of harassment, bias and discrimination seriously and is committed to handling reports in a respectful and timely manner. Your completed reporting form will be sent directly to the University of Dayton Equity Compliance Office and a copy will be forwarded to your email, as provided below.
